The WIAA requires that all student-athletes have a physical every 2 years. All students participating in interscholastic athletics must have this card on file at their school prior to practice or participation. Physical examination taken April 1 and thereafter is valid for the following two school years; physical examination taken before April 1 is valid only for the remainder of that school year and the following school year. This form MUST be signed by both physician and parent.

Athletic Fees
All Dodgeville Middle School Athletes must pay a $30.00 participation fee.
DMS Athletics
All 7th and 8th grade students are invited to participate in our interscholastic athletic program. Sixth grade student are invited to participate in specific sports (Cross Country, Wrestling, and Track & Field)
Sports offered are:
1st Nine Weeks (September and October):

2nd Nine Weeks (November and December)
Basketball - Boys
3rd Nine Weeks (January and February)

The following obligations must be met before a student can participate in interscholastic athletics:
The Code of Conduct for Athletic and Co-Curricular Activities and Concussion Agreement shall be signed by the parent/guardian and returned to the school office.
The user fee paid.
Physical card or an alternate year parent permission card turned into the office.
